Expression of Interest in TeamSports4All

Eligibility checklist — does the child/young person:

Reside in an area where TeamSports4All operates?
Have a parent or guardian who holds a current Health Care Card (HCC), Low Income Health Care Card (LIC/LI), or Pensioner Concession Card (PCC)?
Be named on that concession card (or reside in and be financially dependent on that household)?
Be aged between 5 and 18 years and enrolled in primary or secondary school?
Need financial support to start organised community sport or be at risk of stopping due to financial hardship?

About this referral pathway

This pathway is for children and young people who have participated in a State or National Sporting Organisation event or come and try activity in partnership with TeamSports4All, and where financial hardship may be a barrier to joining their local club.

Please note — we cannot provide an interpreter, but we’re happy to work with your organisation if you are able to provide one.

This referral pathway is reserved for Community Service Organisations working directly with families experiencing financial hardship.

If you are not a Community Service Organisation, referrals cannot be submitted through this form.

Families seeking support should connect with a local Community Service Organisation, school wellbeing team, or community support provider who can assess eligibility and submit a referral where appropriate.

Sporting clubs should use the dedicated partnership enquiry pathway.

Once you complete all three pages of the form, if eligible a TeamSports4All staff member will get in touch with the family to arrange a time to meet them. If you have questions please email hello@teamsports4all.com.au

Sport Availability

Please note sports are dependent on locality and places cannot be guaranteed.

Calisthenics is only available in - Bayside, Frankston and Maroondah.
Gymnastics is only available in Bayside, Kingston, Port Philip and Maroondah.
Martial Arts - only available in very limited capacity.
We are unable to support Swimming, Gym Memberships, Boxing, Golf, Archery, Badminton and any other individual sports.

Our team will review your submission to understand whether TeamSports4All may be able to support your child to participate in community sport. Due to demand and limited funding, we are not able to support every request, however we will aim to contact families where assistance may be possible.

If support is offered, families are expected to apply for any available government sport vouchers (such as the Get Active Kids Voucher) and provide the voucher code to TeamSports4All so it can be applied toward the cost of participation.
